Flagler Health is building a clinical operating system for modern musculoskeletal care, partnering with MSK provider groups and specialty clinics. The company recently raised a $50M Series B and is scaling rapidly.
This is a player-coach role combining hands-on billing operations with team leadership. You will manage a small team of billers, own collections performance across growing partner practices, and serve as the subject matter expert on remote therapeutic monitoring (RTM), chronic care management (CCM), and principal care management (PCM) billing, coding, and payer reimbursement.
Billing Operations & Collections: Own charge and payment reconciliation for new partner practices, establishing clean baselines for collections analysis. Run monthly claim-level payment analysis by date of service, payer, program, and CPT code, flagging unpaid, underpaid, denied, or incorrectly adjusted claims. Lead payer follow-up and denial resolution by researching patterns, contacting payers directly, and recommending corrective actions. Oversee claim entry and corrections across multiple EHR and practice management systems.
Team Leadership: Manage billers and contractors, assigning work, setting priorities, and tracking throughput. Run regular denials and billing team meetings to align on coding rules and standardize documentation. Hold recurring one-on-ones and produce monthly productivity reporting with sample-based quality audits.
Coding Compliance & Documentation: Review claims for ICD-10-CM and CPT compliance, particularly for care management services. Build and maintain diagnosis pairing guides and coding reference materials. Write and maintain SOPs for claim entry workflows across different practice management systems.
Client Billing Support: Lead billing implementation for new partner practices, walking them through RTM/CCM/PCM workflows and EHR template configuration. Serve as billing escalation point for active practices with standing calls and issue resolution. Route feedback upstream to clinical operations and customer success to fix recurring issues at the source.
Process Improvement & Automation: Propose and implement workflow improvements. Partner with engineering to translate coding and billing rules into automation for reconciliation, denial triage, and collections follow-up. Strengthen front-end eligibility processes.
Requirements:
- 5+ years hands-on revenue cycle experience in physician practice or ambulatory billing, including A/R follow-up, denial management, and payment reconciliation
- 2+ years leading or supervising billers, coders, or a denials team
- Working knowledge of CPT and ICD-10-CM coding guidelines, ideally for care management codes (RTM, CCM, PCM)
- Experience with Medicare, Medicare Advantage, and commercial payers, including direct payer representative contact
- Experience across multiple EHR and practice management systems
- Strong spreadsheet skills (pivot tables, lookups) for claim-level analysis
- Clear written communication; comfortable writing SOPs and presenting findings to clients and leadership
Nice to have: CPC, CPB, or CRCR certification; experience working with engineering teams on billing automation; experience at a growth-stage healthcare services company.
