OpenAI's Health team, part of the Personal AGI organization, is seeking an exceptional research engineer to build frontier health capabilities and drive measurable improvements in AI models and products. The role focuses on making high-quality medical intelligence accessible to patients and clinicians at scale.
You will own high-leverage research directions end-to-end, from problem identification and measurement design through experimentation and integration into frontier models. Key responsibilities include developing scalable methods across pretraining, midtraining, reinforcement learning, and post-training to improve health reasoning, knowledge, reliability, and calibration. You'll build and study RL environments grounded in real health problems, create meaningful evaluations that assess whether models genuinely improve outcomes for patients and clinicians, and advance new 0→1 capabilities in health that leverage increasing model intelligence and test-time compute.
The role involves developing models and agents that work with longitudinal, multimodal health data to produce useful predictions, surface scientific insights, and support better individual and population-level decisions. You'll collaborate closely with researchers, engineers, clinicians, and product teams to bring health capabilities into products used by hundreds of millions of people, with a focus on establishing that AI improves real health outcomes rather than just producing impressive demos.
Ideal candidates have an unusually strong track record in machine learning or AI research with depth in at least one critical area: pretraining, reinforcement learning/post-training, evaluations, or frontier biomedical AI problems. You should be a hands-on builder comfortable designing experiments, writing and debugging code, working directly with data and training infrastructure, and moving quickly from idea to evidence. Strong research taste, ability to own ambiguous problems end-to-end, and willingness to do unglamorous work for high-leverage impact are essential. Prior healthcare AI or clinical research experience is a bonus but not required.
The role is based in San Francisco with a hybrid work model of 3 days in-office per week. OpenAI offers relocation assistance.
