Salary: USD 70,000 - 80,000 / annual
Form Health is a virtual-only obesity medicine and digital health clinic seeking an experienced Registered Dietitian Diabetes Specialist to join its cardiometabolic care team. This is a full-time, 40-hour-per-week remote position with flexible scheduling.
You will provide individualized, comprehensive medical nutrition therapy to patients through a telehealth platform, working closely with physicians, endocrinologists, PAs, and NPs. Key responsibilities include:
• Conducting regular video visits with patients to deliver intensive, ongoing nutrition support
• Creating customized nutrition plans tailored to individual patient needs
• Providing diabetes self-management education and counseling, primarily for Type 2 Diabetes
• Serving as a clinical specialist in diabetes care, including interpretation of Continuous Glucose Monitor (CGM) and glucometer data
• Supporting patients with complex comorbidities such as Chronic Kidney Disease and cardiovascular conditions
• Reviewing and providing feedback on patients' daily food photos
• Engaging with patients between visits via text messaging through the Form Health app
• Contributing to the growth and enhancement of the cardiometabolic program in a dynamic environment
You will establish professional, empathetic, and non-judgmental relationships with patients, using evidence-based care and strong interpersonal skills.
Required qualifications: Registered Dietitian credential with 3–5 years of experience in patient counseling, behavior modification, and diabetes care (primarily Type 2). Certified Diabetes Care and Education Specialist (CDCES) certification is highly preferred. Experience in medical weight loss, work with weight loss medications, meal replacement plans, support group facilitation, and cognitive-behavioral therapy counseling techniques is valued.
