Salary: USD 250,000 - 300,000 / annual
Ennoble Care is a mobile primary care, palliative care, and hospice service provider operating across 11 states (NY, NJ, MD, DC, VA, OK, KS, PA, TX, FL, GA). The company brings comprehensive healthcare directly to patients' homes, serving those with chronic conditions and limited mobility through clinician house calls, remote patient monitoring, behavioral health management, and chronic care management programs.
The Regional Chief Medical Officer (CRMO) will lead the medical component of Ennoble Care's home-based primary and palliative care programs within a designated multi-state region (NJ & PA). This is a full-time executive leadership role combining clinical practice with strategic medical oversight.
The position comprises three integrated functions: (1) independent clinical physician providing direct patient care (~60% of time), (2) House Calls medical director overseeing clinical operations and provider supervision, and (3) regional leadership and administration (~40% of time).
Clinical Practice Responsibilities: Provide high-quality, evidence-based, patient-centered care to a small patient panel through house calls and palliative care services. Function as an independent medical provider (internal medicine, family medicine, geriatrics, or palliative medicine specialty). Deliver direct patient care including medical histories, physical examinations, differential diagnosis, treatment regimens, and chronic disease management. Ensure timely clinical documentation and home health order completion.
House Call Medical Director Services: Oversee medical and administrative services provided by physicians, nurse practitioners, and physician assistants. Provide clinical direction through regular chart review and development of clinical guidelines and best practices. Serve as collaborating or supervising physician as required by state regulations. Act as trusted consultant for clinical escalations. Ensure continuity of care during provider vacancies. Provide field training and clinical assessments for new and underperforming providers. Assist in improving care delivery processes, protocols, and quality assurance. Provide after-hours backup call coverage for escalations and complex cases. Contribute to EHR software design and testing.
Regional Leadership and Administration: Provide strategic medical leadership across the multi-state region. Coordinate with local clinical leadership to ensure consistent care standards. Participate in performance improvement and quality initiatives. Support market growth, patient panel expansion, and operational profitability. Collaborate with regional administrators and executive leadership on business development. Champion continuous improvement of core programs (behavioral health integration, remote patient monitoring, GUIDE, ACO programs). Ensure compliance with Medicare/Medicaid requirements, Stark Law, Anti-Kickback Statute, and applicable healthcare regulations across all regional jurisdictions. Represent Ennoble Care at professional conferences, meetings, and community events.
Job Conditions: Combination of office-based, home-visit, and remote work across multiple states. Requires moderate physical activity including sitting, standing, stooping, walking, and travel by car. Face-to-face patient visits required with moderate physical exertion. Travel required by car or airplane to regional locations, seminars, conferences, and meetings. Position may require after-hours availability for clinical escalations and emergencies. Must meet deadlines required by governing and regulatory organizations.
QUALIFICATIONS
Required:
• Doctor of Medicine (MD) or Doctor of Osteopathy (DO) degree from an accredited institution
• Board Certification by the American Board of Internal Medicine, American Board of Family Medicine, or equivalent specialty board
• Active, unrestricted medical license in good standing in all states within the designated region (or ability to obtain within 90 days)
• Current Drug Enforcement Administration (DEA) controlled substances registration
• Minimum of 5 years of clinical experience in primary care, geriatric medicine, palliative care, or hospice medicine
• Minimum of 3 years of medical leadership or director-level experience
• Training and experience in the psychological and medical needs of homebound older adults living with chronic illness
• Not excluded from participation in Medicare or other Federal Health Care Programs
• Demonstrated ability to communicate effectively, both verbally and in writing
• Valid driver's license and ability to travel within the region
Preferred:
• Board certification in Geriatrics or Palliative Medicine
• Experience with value-based care models and quality metrics
• Experience with home-based primary care delivery models
• Prior experience in multi-site or multi-state healthcare leadership
• Fellowship training in geriatric medicine or palliative care
