Salary: USD 162,010 - 205,433 / annual
WelbeHealth PACE (Program of All-Inclusive Care for the Elderly) is seeking a Primary Care Provider (Nurse Practitioner or Physician Assistant) to deliver comprehensive medical care to seniors in a value-based, team-oriented setting.
In this role, you will work under the direction of the Medical Director and collaborate with WelbeHealth physicians and an interdisciplinary team (IDT) to provide holistic care to elderly participants. You'll manage a smaller patient caseload than traditional primary care settings, enabling you to build long-term relationships and practice true value-based medicine.
Key responsibilities include:
- Provide continuous medical coverage and perform comprehensive in-person assessments (initial, semi-annual, and unscheduled) for participants
- Collaborate with physicians and IDT members to develop individualized care plans and ensure effective care coordination
- Oversee discharge planning from acute or long-term care settings
- Make regular visits to participants in skilled nursing facilities and residences as needed
- Review outcomes and utilization data (hospitalizations, specialist usage) to establish best practices and drive policy improvements
- Educate participants and families about their care

Requirements:
- Graduation from an accredited school of nursing
- Current certification as a Nurse Practitioner or Physician Assistant with prescriptive authority
- Active RN state license
- Minimum 2 years of experience working with frail or elderly populations
- Current knowledge of chronic care, geriatric issues, and best practices
- Reliable transportation
- Willingness to participate in rotating night and weekend on-call coverage
- Ideal candidates have experience caring for seniors with complex care needs (Medicare/Medicaid populations)
