Salary: USD 23 - 30 / hourly
Devoted Health is seeking a Pharmacy Care Coordinator to support clinical pharmacy operations and medication management for Medicare members. The role is part of Devoted's mission to improve health outcomes for older Americans by ensuring members receive evidence-based medication therapies and coordinated care.
In this position, you will serve as a critical link between clinical pharmacists, prescribers, pharmacies, and members. You will conduct outbound member outreach via phone and portal chat to schedule pharmacy appointments, identify medication gaps, and address clinical concerns. You will manage an assigned caseload independently, overseeing follow-ups with providers and pharmacies on medication recommendations, therapy changes, and claims issues. You will help members navigate their Devoted Health benefits and participate in cost-of-care initiatives by connecting members and providers with lower-cost medication alternatives.
The role supports Devoted's quality and Medicare Stars performance across multiple programs including SUPD (Statin Use in Persons with Diabetes), SPC (Statin Use in Persons with Heart Disease), MTM (Medication Therapy Management), and polypharmacy management. You will document all interactions thoroughly, maintain clear next steps, and communicate proactively with the broader pharmacy and operations teams. You may also assist with training new team members, participate in pilots and special projects, and contribute to process improvements.

REQUIREMENTS:
- At least 3 years of work experience in healthcare or retail pharmacy
- Active National PTCB Certification (required)
- Comfortable making high-volume outbound calls to members, providers, and pharmacies
- Ability to manage daily responsibilities and tasks independently
- Ability to shift between tasks, projects, and priorities frequently
