Salary: USD 41,600 - 54,600 / annual

Form Health is a national telemedicine weight loss clinic specializing in obesity medicine. The company delivers research-based treatment through telehealth, combining long-term physician care, dietitian support, and FDA-approved medications. Founded in 2019 and venture-backed, Form Health is scaling rapidly as obesity medicine enters a period of growth. The Patient Care Coordinator role is central to clinical operations and patient experience. You will serve as the primary point of contact between patients and their care team (physicians and registered dietitians), managing communication flow, triaging clinical messages, and conducting regular outgoing patient communications to ensure engagement and program success. Key responsibilities span four areas: **Care Team Management:** Act as the primary contact maintaining communication between patients and clinicians. Receive, triage, and resolve incoming clinical messages. Conduct regular outgoing patient communications. Provide routine administrative support to clinicians and contribute to special projects that improve systems and care efficiency. **Lab & Medical Records Support:** Collect and request medical records from primary care or other providers. Review patient-uploaded records. Facilitate medical records and prescriptions, including sending lab requisitions. Assist with prior authorization coordination, medication refills, pharmacy questions, and support clinicians with pre-visit lab orders. **Membership & Program Status:** Guide new patients through the program and address general questions. Support account changes and cancellation requests. Provide feedback to Product, Engineering, and Marketing teams to improve tools and care. Assist with clinical data collection for performance tracking and quality improvement. Provide scheduling and appointment support. **Billing & Insurance:** Assist with general insurance and billing questions. Provide information on coverage updates, address billing concerns, coordinate with the billing department, answer membership change questions, provide cost information, and answer eligibility questions. You will work in a 100% remote environment, collaborating with an experienced clinical team passionate about helping patients improve their health through weight loss. **Requirements:** - Medical Assistant certification required: Certified Medical Assistant (CMA) from AAMA, OR Registered Medical Assistant (RMA) from AMT, OR Certified Clinical Medical Assistant (CCMA) from NHA - At least 2 years of experience in a fast-paced primary care, obesity medicine, or endocrinology practice - Experience with prescription prior authorization processes (preferred) - Excellent customer service skills - Excellent verbal and written communication skills - Highly organized with proven ability to manage multiple tasks and time effectively - Proficient computer skills, including Slack and Google Workspace, with ability to quickly adapt to custom-built IT systems
