As an RN Case Manager based in Harrisburg, PA, you will oversee comprehensive care coordination for a diverse patient caseload, reporting directly to the Director of Nursing. This is a high-agency clinical role requiring strong advocacy, leadership, and organizational skills.
Key responsibilities include:
- Managing diverse patient caseloads and collaborating with primary care physicians, therapy providers, families, and internal support teams to drive optimal clinical outcomes
- Providing clinical oversight and coaching to Certified Nursing Assistants (CNAs) and Home Health Aides (HHAs) to ensure adherence to clinical standards
- Developing, implementing, and adjusting individualized care plans based on patient-specific assessments and health goals
- Maintaining timely and accurate electronic documentation with real-time charting and minimum 24-hour turnaround compliance
- Educating and supporting patients and families on treatment plans and healthcare needs
- Identifying and escalating clinical issues, order management concerns, and patient emergencies to Clinical Leads
- Participating in on-call rotations (approximately one weekend per month), holiday coverage, and weekday phone triage
- Monitoring and improving patient readmission rates while maintaining high caregiver and patient satisfaction
- Managing administrative tasks including mileage reimbursements and visit documentation accuracy
Success in this role means managing an optimized patient caseload with consistent documentation standards, building strong relationships with patients, families, and external providers, and demonstrating measurable improvements in satisfaction and reduced readmission rates within 12 months.
The role is full-time hybrid based in Harrisburg, PA, requiring local travel to patient locations and scheduling flexibility including weekends and on-call participation.
REQUIREMENTS:
- Education: Associate's Degree in Nursing or Bachelor of Science in Nursing (BSN)
- Licensure: Current, valid, unencumbered Registered Nurse (RN) license in Pennsylvania or valid Compact/Multi-state RN License
- Experience: Minimum 2 years of clinical experience as a Registered Nurse
- Transportation: Reliable transportation, valid driver's license, and current auto insurance
- Scheduling: Ability to work flexible hours, travel locally, and participate in on-call and weekend rotations
- Location: Must reside in or be willing to permanently relocate to Harrisburg, PA area
PREFERRED QUALIFICATIONS:
- 1+ years of home health experience
- Prior clinical case management or supervisory experience
- Experience in pediatric or adult home care
- Understanding of Medicaid paid family caregiving programs and CNA scope of practice
- Familiarity with OASIS documentation standards (training provided if needed)
