Hopscotch Primary Care is transforming healthcare in rural America by providing high-quality, accessible primary care. The company operates a value-based care model serving thousands of patients across rural communities, addressing the gap where nearly 20% of Americans live in rural areas but only 11% of physicians practice there.
As an Outreach Leader, you will drive patient acquisition and community engagement for Hopscotch's primary care clinics. This is a field-based sales role with significant autonomy to build your territory and execute outreach strategy.
Key responsibilities include:
**Drive Patient Growth:**
- Own patient acquisition goals within your assigned territory in Buncombe County
- Build and execute a territory sales plan to achieve monthly and quarterly enrollment targets
- Prospect for new opportunities through networking, cold outreach, community engagement, and referrals
- Identify, qualify, and convert prospective patients into active Hopscotch patients
- Develop and maintain a healthy pipeline of leads and manage opportunities through every stage of the enrollment process
- Track sales activity, conversion metrics, and pipeline performance using CRM tools
**Build Community Partnerships:**
- Develop strong relationships with community organizations, senior centers, churches, employers, healthcare providers, pharmacies, and other referral sources
- Identify new partnership opportunities that increase awareness and patient referrals
- Serve as the face of Hopscotch Primary Care throughout your territory
- Represent Hopscotch at community events, health fairs, educational programs, and networking opportunities
**Relationship Management:**
- Build lasting relationships with patients, caregivers, and community partners
The position is home-based in the greater Asheville, NC area and requires daily local travel throughout Buncombe County.
**Requirements:**
- High school diploma or equivalent (required)
- Valid driver's license and reliable transportation (required)
- U.S. work authorization (required)
- Bachelor's degree (preferred)
- Healthcare, Medicare, insurance, or value-based care experience (preferred)
- Experience using a CRM such as Salesforce (preferred)
- Experience working with older adults or community organizations (preferred)
