Ennoble Care is a multi-state mobile primary care, palliative care, and hospice service provider serving patients across 15+ states including Ohio, New York, New Jersey, Pennsylvania, Maryland, Virginia, Florida, Texas, and others. The company delivers in-home and facility-based care for older adults with chronic conditions and limited mobility.
You will serve as Medical Director for Geriatric Medicine in the Youngstown, Ohio region, reporting to the Regional Chief Medical Officer. This is a hybrid clinical and leadership role combining hands-on patient care with medical direction responsibilities. The position offers flexible scheduling from a minimum of two days per week to full-time, accommodating physicians seeking to apply deep geriatric expertise in a growing home-based care organization.
Clinical Responsibilities:
- Provide direct medical care to a panel of older adults across clinic, long-term care, assisted living, and home settings
- Conduct comprehensive geriatric assessments including medical, functional, cognitive, and psychosocial evaluations
- Diagnose and manage complex chronic conditions common in older adults (dementia, frailty, cardiovascular disease, osteoporosis, diabetes)
- Monitor and adjust medications with attention to polypharmacy and age-related pharmacologic considerations
- Develop individualized treatment plans emphasizing prevention, health maintenance, and patient/caregiver education
- Guide patients and families through end-of-life care planning, advance directives, and palliative care coordination
- Coordinate care with multidisciplinary teams including nurses, social workers, therapists, pharmacists, and specialists
Medical Direction & Leadership:
- Collaborate with and support nurse practitioners and physician assistants on shared patient panels
- Support the Regional CMO in developing clinical protocols and best practices for geriatric care
- Contribute geriatric expertise to case reviews and provider training
- Participate in quality improvement initiatives and clinical research
- Maintain timely, accurate clinical documentation in compliance with regulatory and organizational standards
- Maintain continuing medical education relevant to geriatric and home-based primary care
Requirements:
- MD or DO from an accredited institution
- Board certification (or eligibility) in Internal Medicine or Family Medicine
- Active, unrestricted Ohio medical license (or ability to obtain)
- Strong clinical skills in diagnosing and managing complex, age-related conditions
- Fellowship training in Geriatric Medicine preferred
- Experience in long-term care, skilled nursing facilities, home health, or palliative care strongly preferred
- Familiarity with interdisciplinary care models and comfort working with nurse practitioners, physician assistants, and care teams
- Knowledge of geriatric syndromes, mobility and fall-risk assessment, and cognitive evaluation tools
- Not excluded from participation in Medicare or other Federal Health Care Programs
- Excellent verbal and written communication skills with strong empathy and teamwork abilities
- Valid driver's license and ability to travel locally in Youngstown area
