The Medical Director, Geriatric Medicine role is based in the Youngstown, Ohio area and reports to the Regional Chief Medical Officer. This is a hybrid position with flexible scheduling—minimum two days per week up to full-time, depending on candidate availability and business needs. The role combines hands-on geriatric clinical practice with medical leadership responsibilities.
Key responsibilities include:
• Providing direct medical care to a panel of older adults across clinic, long-term care facilities, assisted living communities, and home settings, with a focus on patient-centered geriatric medicine.
• Conducting comprehensive geriatric assessments covering medical, functional, cognitive, and psychosocial dimensions to guide individualized care planning.
• Diagnosing and managing complex chronic conditions common in older adults—dementia, frailty, cardiovascular disease, osteoporosis, diabetes—with particular attention to polypharmacy and age-related pharmacologic considerations.
• Developing individualized treatment plans emphasizing prevention, health maintenance, and patient/caregiver education.
• Guiding patients and families through end-of-life care planning, advance directives, and palliative care coordination.
• Coordinating care with Ennoble Care's multidisciplinary team (nurses, social workers, therapists, pharmacists, specialists) and communicating effectively with patients, families, and caregivers on goals of care.
• Collaborating with and supporting nurse practitioners and physician assistants on shared patient panels.
• Supporting the Regional CMO in developing clinical protocols, best practices for geriatric care, case reviews, and provider training.
• Participating in quality improvement initiatives and clinical research as directed.
• Maintaining continuing medical education relevant to geriatric and home-based primary care.

QUALIFICATIONS:
• Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) from an accredited institution, with board certification (or eligibility) in Internal Medicine or Family Medicine.
• Active, unrestricted Ohio medical license (or ability to obtain).
• Strong clinical skills in diagnosing and managing complex, age-related conditions.
• Fellowship training in Geriatric Medicine is preferred.
• Experience in long-term care, skilled nursing facilities, home health, or palliative care is strongly preferred.
• Familiarity with interdisciplinary care models and comfort working closely with nurse practitioners, physician assistants, and other care team members.
• Knowledge of geriatric syndromes, mobility and fall-risk assessment, and cognitive evaluation tools.
• Not excluded from participation in Medicare or other Federal Health Care Programs.
• Excellent verbal and written communication skills, with strong empathy and teamwork abilities.
• Valid driver's license and ability to travel locally to patients and facilities in the Youngstown, Ohio area.
