Ennoble Care is a multi-state mobile primary care, palliative care, and hospice provider serving patients across 15+ states. The company delivers in-home and facility-based care for older adults with chronic conditions and limited mobility, emphasizing a continuum of care model with remote patient monitoring, behavioral health, and chronic disease management.
You will serve as Medical Director for Geriatric Medicine in the Youngstown, Ohio region, reporting to the Regional Chief Medical Officer. This role uniquely blends hands-on clinical practice with medical leadership responsibilities. You will provide direct geriatric care to a patient panel across multiple settings—clinics, long-term care facilities, assisted living communities, and patients' homes—while supporting the region's broader medical direction, quality initiatives, and care coordination efforts.
Key responsibilities include:
- Delivering direct patient care with comprehensive geriatric assessments (medical, functional, cognitive, psychosocial)
- Diagnosing and managing complex chronic conditions in older adults (dementia, frailty, cardiovascular disease, osteoporosis, diabetes) with attention to polypharmacy and age-related pharmacology
- Developing individualized treatment plans emphasizing prevention and patient/caregiver education
- Guiding end-of-life and palliative care planning, advance directives, and goals-of-care conversations
- Coordinating care across Ennoble's multidisciplinary team (nurses, social workers, therapists, pharmacists)
- Collaborating with and supporting nurse practitioners and physician assistants on shared patient panels
- Supporting the RCMO in developing clinical protocols, best practices, and geriatric-specific training
- Participating in quality improvement initiatives and clinical research
- Maintaining continuing medical education in geriatric and home-based primary care
The position offers flexible scheduling: minimum two days per week to full-time, based on candidate availability and business needs. This is ideal for a physician seeking to apply deep geriatric expertise in a growing home-based care organization.
REQUIREMENTS:
- MD or DO from an accredited institution
- Board certification (or eligibility) in Internal Medicine or Family Medicine
- Active, unrestricted Ohio medical license (or ability to obtain)
- Strong clinical skills in diagnosing and managing complex, age-related conditions
- Fellowship training in Geriatric Medicine (preferred)
- Experience in long-term care, skilled nursing facilities, home health, or palliative care (strongly preferred)
- Familiarity with interdisciplinary care models and comfort collaborating with NPs, PAs, and care teams
- Knowledge of geriatric syndromes, mobility/fall-risk assessment, and cognitive evaluation tools
- Not excluded from participation in Medicare or other Federal Health Care Programs
- Excellent verbal and written communication skills with strong empathy and teamwork abilities
- Valid driver's license and ability to travel locally in the Youngstown area
