Salary: USD 165,000 - 220,000 / annual

Outro Health is a venture-backed healthtech company pioneering medication deprescribing—helping patients safely reduce psychiatric medications through specialist clinical care and purpose-built software. Founded by Dr. Mark Horowitz, a leading deprescribing expert, the company has achieved real revenue and partnerships with major health systems in its first year. You will be Outro's senior hands-on engineering leader, taking primary ownership of the engineering function as the founding engineer transitions to a Product Lead role. This is a deeply technical position: you will write a substantial share of production code, make critical architectural decisions, and ensure systems remain coherent, reliable, and secure. Your scope spans the full stack: clinician tools and patient-facing products, medication and clinical data systems, insurance and healthcare integrations, internal operations, and infrastructure. You will own technical direction, architecture, engineering standards, HIPAA compliance, SOC 2, access controls, and data privacy—areas where software reliability directly impacts patient safety. As Outro scales, you will hire and mentor subsequent engineers, introduce process without bureaucracy, and grow into Head of Engineering. The role remains hands-on: you will work closely with the Product Lead, founders, clinicians, and operators in a small, high-ownership environment. The tech stack is modern and TypeScript-forward: Node.js, Next.js, React, React Native, Postgres, BigQuery, GCP, and Terraform. You should have shipped substantial production systems, strong judgment across architecture and data modeling, real product instinct, and high standards for reliability in regulated or healthcare contexts. Healthcare experience is useful but not required; sound judgment and ability to learn complex domains quickly matter more.
