Ennoble Care is a mobile primary care, palliative care, and hospice service provider operating across 11 states (NY, NJ, MD, DC, VA, OK, KS, PA, TX, FL, GA). The company brings clinical care directly to patients' homes, offering a continuum of care for those with chronic conditions and limited mobility, including remote patient monitoring, behavioral health management, and chronic care management.
The Hospice Clinical Director will oversee the coordination and management of patient care for the Philadelphia region, working closely with the Executive Director to maintain quality standards and ensure compliance with agency objectives and policies.
Key responsibilities include:
- Coordinate and oversee direct and indirect patient services provided by clinical personnel
- Provide supervision and guidance to clinical staff to ensure quality of services and maintenance of standards
- Perform chart audits and gather data for QAPI (Quality Assurance and Performance Improvement) activities
- Appraise clinical staff performance during probationary periods and annually; provide teaching and counseling to enhance service delivery and professional development
- Evaluate patient needs and appropriateness of home care through intake screening review; assist with staff orientation
- See patients in home settings as needed; maintain adequate medical supplies
- Ensure Medicare, state, and ACHC (Accreditation Commission for Health Care) regulations are followed by all clinical staff
- Assist Executive Director with additional responsibilities as requested
- Pursue ongoing professional development through formal education, workshops, conferences, and professional organization participation
The role is primarily desk-based with occasional field visits to patient homes for supervision. Physical demands include minimal lifting of supplies and charts, occasional bending/stooping, and the ability to access patients' homes (which may not be wheelchair accessible). Travel to and from patient homes is required when supervision is scheduled.
QUALIFICATIONS:
- Graduate of an accredited School of Nursing
- Registered Nurse licensed by the state of Pennsylvania
- Minimum 2 years of nursing experience in home hospice or skilled home care (hospice preferred)
- At least 1 year of supervisory experience
- Ability to market and deal tactfully with customers and the community
- Knowledge of business management, governmental regulations, and ACHC standards
- Strong verbal and written communication skills
- Ability to demonstrate self-confidence, positive attitude, and maintain commitment and enthusiasm toward goal achievement
