HeartFlow is a publicly traded medical technology company (HTFL) pioneering AI-driven solutions for coronary artery disease diagnosis and management. The company's flagship product, HeartFlow FFR_CT Analysis, is an AI-powered non-invasive cardiac test that provides 3D visualization of coronary arteries and blood flow assessment. Used by over 500,000 patients worldwide and cleared in the US, UK, Europe, Japan, and Canada, HeartFlow combines clinical expertise with cutting-edge AI technology.
We are seeking an Engineering Manager to build and lead our new Agentic AI initiatives team. This is a unique "player-coach" role that begins with deep hands-on technical work and evolves into full management as the team scales.
In the initial phase (6-12 months), you will act as the lead architect and foundational engineer, designing and implementing a production-grade agentic AI platform from scratch. You will establish core patterns for multi-agent orchestration, state management, tool usage, and memory systems. This includes designing robust, secure, and scalable infrastructure that translates HeartFlow's decade of clinical AI success into autonomous medical technology.
As the team grows, you will transition into dedicated management, responsible for scaling a world-class engineering team, guiding execution, and shaping long-term technical strategy.
Key responsibilities include: architecting and coding the agentic AI platform; building and leading the engineering team; driving technical execution of agent-driven product features from ideation to FDA-cleared production deployment; implementing advanced safety guardrails and compliance frameworks for regulated healthcare software; collaborating cross-functionally with product, clinical experts, and data scientists; and championing software engineering best practices including CI/CD, comprehensive testing, and logging for non-deterministic systems.
You will have a true blank-canvas opportunity to design systems that parse clinical requirements, map software modules, and auto-generate test cases—all while maintaining the safety and determinism required for FDA-cleared medical software.
Required: 8+ years professional software engineering experience with proven track record shipping production software; 5+ years managing, mentoring, or leading engineering teams; deep hands-on experience building LLM applications and understanding agentic architectures, orchestration frameworks, and vector databases; advanced proficiency in Python or Go; solid grasp of microservices, API design, and cloud infrastructure (AWS/GCP); player-coach mentality with willingness to code while building team; passion for greenfield projects and solving complex open-ended problems.
Nice-to-have: experience deploying AI in regulated industries (healthcare, finance) or navigating HIPAA/GDPR; open-source LLM/agentic AI contributions; advanced prompt engineering, fine-tuning, or RAG optimization experience.
