Aledade seeks a Director of Population Health Impact to lead the development and implementation of healthcare equity initiatives across the organization. This role is responsible for stewarding the company's commitment to delivering equitable clinical outcomes and transforming the healthcare experience for underserved and historically marginalized communities.
The Director will partner with cross-functional teams throughout the organization to meet and exceed goals for decreasing healthcare disparities and realizing health equity in the communities served. Key responsibilities include leading investigative data analysis, developing solutions, and executing key initiatives and projects that drive measurable continuous improvement in clinical and financial healthcare equity outcomes.
The role involves collaborating with other leaders to develop and implement processes, pilots, and partnerships designed to reduce healthcare inequity and disparities. The Director will be accountable for ensuring all initiatives align with the organization's clinical and financial healthcare equity goals while driving sustainable, measurable impact.
