Salary: USD 61.21 - 78 / hourly
WelbeHealth PACE is a value-based healthcare program serving vulnerable seniors, enabling them to remain in their homes and communities with comprehensive medical care and community-based services.
The Case Manager, RN is a clinical role that directly guides participant care as a key member of the interdisciplinary team (IDT). Reporting to clinical leadership, you will deliver and oversee all aspects of care coordination, including conducting PACE assessments, developing personalized participant care plans, and supporting care transitions.
Day-to-day responsibilities include:
- Conducting clinic nursing assessments to determine participant needs, preferences, and goals
- Actively participating in IDT meetings to develop and refine participant care plans
- Delivering and documenting nursing interventions per care plans, including maintaining safe environments, responding to physician orders, and administering medications
- Collaborating with the WelbeHealth PACE Physician and other IDT members to coordinate care
- Communicating with participants and families to revise care plans as needed
- Instructing participants, families, and caregivers on disease processes, self-care techniques, and prevention strategies
The role offers a Monday-through-Friday schedule with no weekends or on-call duties, enabling relationship-building with participants rather than short-term care delivery. Requirements:
- Graduate of an accredited school of nursing; Bachelor of Science (BSN) preferred
- Unencumbered California RN License
- Current Basic Life Support (BLS) and First Aid Certification required
- Two (2) years of nursing experience in a clinical setting with a frail or elderly population preferred
- Must obtain CPR and First Aid certification within 180 days of hire and maintain thereafter (CPR and First Aid certification upon hire preferred)
- Ideally, experience working with frail and elderly populations
