Pelago Health is seeking a Care Specialist to join its digital clinic and serve as the first clinical point of contact for members entering the substance use care program. This is a direct-service, remote clinical role based anywhere in the USA.
The role is approximately 70% dedicated to member onboarding activities, including intake assessments, welcome calls, and care coordination. The remaining 30% focuses on maintaining a small ongoing caseload and providing flexible clinical support through temporary member coverage, pilot initiatives, and operational needs.
Key responsibilities include:
- Conducting initial screenings and intake assessments to evaluate behavioral health history, substance use, symptoms, and member goals
- Creating a welcoming, supportive onboarding experience as the first clinical point of contact
- Understanding all Pelago care pathways and guiding members toward the most appropriate level of care
- Facilitating warm transitions by connecting members with assigned counselors or coaches
- Conducting welcome calls and early member touchpoints to build rapport and increase engagement
- Maintaining a small ongoing caseload by providing individual counseling and monitoring progress
- Providing temporary member coverage during clinician leave or staffing transitions
- Supporting special projects and pilot programs
- Documenting member interactions and contributing to quality improvement efforts
- Utilizing evidence-based approaches including CBT, Motivational Interviewing, mindfulness-based interventions, harm reduction, and relapse prevention
- Collaborating with cross-functional teams to optimize member experience
- Maintaining active licensure and regulatory compliance
Required qualifications:
- 3+ years of clinical experience working with individuals with substance use disorders
- Demonstrated expertise in CBT, Motivational Interviewing, and relapse prevention
- Active and unrestricted state credential (license/certification) from a state credentialing board
- IC&RC or NAADAC credentialing (e.g., AADC, CAADC, LADC, LCSW with SUD treatment scope)
- Bachelor's degree in a relevant field
Preferred qualifications include SBIRT care experience, excellent attention to detail, program development and quality assurance experience, strong communication and organizational skills, technological proficiency with virtual care tools, and research/data analysis experience. Candidates must have a reliable home office setup with 50–100 Mbps internet bandwidth and a confidential workspace.
