Habitat Health is a mission-driven healthtech company expanding access to comprehensive care for older adults through the Program of All-Inclusive Care for the Elderly (PACE). Backed by leading investors including NEA and Kaiser Permanente, the company is in a period of significant growth. The Broker Relations Manager owns the full lifecycle of Habitat Health's relationships with Field Marketing Organizations (FMOs) and insurance brokers—the primary referral channel for enrolling Medicare-eligible seniors into PACE programs. This is a hybrid role based in California (Sacramento, East Bay, or LA County), requiring up to 25% travel to PACE centers and broker events. Key responsibilities include: developing and executing broker channel strategy to meet referral and enrollment targets; recruiting Medicare agents, brokers, and FMOs across service areas; providing onboarding, training, and ongoing support to ensure brokers accurately represent PACE and comply with all regulatory requirements; building productive relationships through regular communication, field visits, and events; coordinating required compliance and marketing training; tracking referrals through enrollment; and reporting broker performance and market trends to Growth leadership. The ideal candidate brings 3+ years of relevant experience recruiting, contracting, onboarding, and managing insurance agents or FMOs—ideally in healthcare delivery at a high-growth startup, operations, program management, or healthcare payer/provider consulting. A bachelor's degree in business, healthcare administration, marketing, or related field is required. Strong project management, relationship-building, communication, and organizational skills are essential. Experience with Medicare, Medicaid, Medicare Advantage, dual-eligible populations, or PACE is a plus. The role demands comfort with ambiguity, a growth mindset, integrity, and alignment with Habitat Health's mission-driven values.
