Devoted Health is seeking an Active Care APP Manager to lead and build out the clinical and operational oversight of a team of Advance Care Providers (APPs) delivering primary care to Devoted Health Plan members through a virtual, AI-enabled platform. This is a hybrid clinical and management role launching in Summer/Fall 2026.
You will report directly to the Medical Director for Primary Care and serve as a crucial contributor to the buildout of Devoted's delivery model. Active Care is a core team within primary care, responsible for stabilizing and managing complex patients with multiple chronic conditions, uncontrolled chronic disease, and preventative health needs in a longitudinal care model.
Key responsibilities include:
**Team Leadership & Development**: Provide ongoing coaching to support clinical reasoning, member communication, and documentation quality. Conduct regular 1:1s, performance reviews, and develop individualized coaching plans. Foster a team environment rooted in transparency, shared learning, and psychological safety.
**Performance & Quality Oversight**: Monitor performance trends and visit quality using dashboards, documentation review, and direct observation. Identify performance variation early and address gaps collaboratively. Ensure consistent workflow adherence, productivity expectations, and quality of member interactions. Develop and refine standard operating procedures (SOPs) aligned with program growth.
**Program Design & Collaboration**: Collaborate with primary care leadership to define Active Care Service features and design new workflows. Lead implementation of workflows and monitor effectiveness. Foster interdisciplinary cross-team collaboration between Active Care and seven other supporting teams.
**Operational & Scheduling Support**: Monitor team capacity, schedule coverage, visit volume patterns, and PTO to ensure stable access to care. Support onboarding and orientation of new clinicians. Participate in interviewing and selection of APP candidates. Engage in process improvement initiatives.
**Clinical Responsibilities**: Maintain a limited panel of weekly clinical visits, flexing based on staffing and program needs. Coordinate care with interdisciplinary teammates and external providers. Utilize Devoted's internal clinical systems and provide feedback for tool and workflow improvements.
Required: 3+ years NP clinical experience in primary care or relevant settings, demonstrated experience leading change, strong track record supporting and mentoring clinicians, stellar communication skills, and comfort navigating complex healthcare environments.
