Salary: USD 145,399 - 191,927 / annual
WelbeHealth is seeking a Program Director to oversee critical PACE (Program of All-inclusive Care for the Elderly) operations at their San Bernardino day center. Reporting to the Executive Director, you will manage multiple clinical and non-clinical service lines including nutrition, social work, day center operations, personal care services, scheduling, and participant experience.
Key responsibilities include hiring, training, and supervising a multi-disciplinary team across nursing, social work, and rehabilitation services. You will develop and facilitate an effective Interdisciplinary Team (IDT) to coordinate participant care, ensure adherence to Standard Operating Procedures and regulatory requirements, and drive quality outcomes. You'll oversee site operations, supplies, equipment management, and vendor relationships while collaborating with shared services including home care, home health, transportation, and utilization management.
Required qualifications: Bachelor's degree in health administration, healthcare management, nursing administration, gerontology, social work, or business administration (master's preferred). Minimum 7 years of experience in senior healthcare program management, with at least 3 years in a supervisory role. Professional background in nursing, social work, psychology, or therapy with demonstrated competence working with frail elderly. Multi-disciplinary management experience required. CPR and First Aid certification must be obtained within 180 days of hire. Value-based care experience preferred.
