Salary: USD 145,399 - 191,927 / annual
WelbeHealth is seeking a Program Director to oversee critical PACE (Program of All-inclusive Care for the Elderly) operations at their Pasadena day center. Reporting to the Executive Director, you will manage multiple clinical and non-clinical functions including nutrition, social work, day center operations, personal care services, scheduling, and participant experience.
Key responsibilities include hiring, training, and supervising staff across multiple disciplines (nursing, social work, rehabilitation); developing and facilitating an effective Interdisciplinary Team (IDT) to coordinate participant care; ensuring adherence to Standard Operating Procedures and regulatory requirements; collaborating with shared services (home care, home health, transportation, utilization management) to achieve high participant outcomes; and overseeing facility management, supplies, equipment, and vendor relationships.
You will lead a multi-disciplinary team focused on enabling seniors to remain independent in their own homes while receiving comprehensive support and clinical services. The role requires strong operational management skills, healthcare industry knowledge, and the ability to coordinate complex care delivery across multiple service lines.
Required qualifications: Bachelor's degree in health administration, healthcare management, nursing administration, gerontology, social work, or business administration (Master's preferred); minimum 7 years of healthcare management experience with at least 3 years in a supervisory role managing a senior health program, outpatient clinic, or healthcare facility; professional background in nursing, social work, psychology, or therapy with demonstrated competence working with frail elderly; multi-disciplinary management experience; CPR and First Aid certification required within 180 days of hire (preferred upon hire). Value-based care experience preferred.
