Salary: USD 70,000 - 80,000 / annual
Devoted Health seeks a Primary Care Social Work Case Manager to join its interdisciplinary virtual care team serving older Americans with chronic conditions. This remote role provides telephonic and video-based social work case management support, addressing the psychosocial and social determinants of health for high-needs patients in the primary care program.
Key responsibilities include conducting psychosocial screenings and developing personalized care plans; facilitating telephonic and video visits with patients and caregivers; collaborating with clinicians, nurses, and care coordinators on interdisciplinary rounds and panel reviews; navigating behavioral health, long-term care planning, and advance care planning; and documenting clinical interactions in the electronic health record. You will establish trust-based relationships with vulnerable populations, help families understand patient needs, balance competing priorities, and participate in team huddles and program development.
Required qualifications: Master's degree in social work with active license; minimum 2 years medical field experience; comfort with technology and remote work; strong interpersonal and communication skills; ability to engage patients via video and phone in a fast-paced environment.
Desired qualifications include bilingual fluency (Creole, Spanish, or Vietnamese); health insurance or Medicare Advantage experience; geriatrics background.
