The Outreach & Enrollment Supervisor leads Habitat Health's Outreach and Enrollment team, driving participant growth through strategic leadership, operational excellence, and talent development. This role oversees end-to-end outreach, intake, and enrollment functions, ensuring prospective participants receive a seamless, participant-centered experience from initial engagement through enrollment. Key responsibilities include: leading, coaching, and developing a high-performing Outreach and Enrollment team by fostering accountability, professional growth, continuous learning, and a culture of belonging; developing and executing outreach and enrollment strategies that drive participant growth, strengthen referral partnerships, and support organizational objectives; monitoring team performance, operational metrics, and enrollment outcomes using data and insights to identify improvement opportunities; partnering closely with center leadership and cross-functional teams to remove operational barriers and leverage data-driven insights; and ensuring compliance with applicable regulatory requirements. The role requires strong people management capabilities, strategic thinking around growth and partnerships, operational acumen, and a commitment to serving older adults with dignity and person-centered care. The estimated start date is October 5, 2026.
