Salary: USD 280,000 - 305,000 / annual

Devoted Health is building a national Musculoskeletal (MSK) clinical program for seniors, leveraging their AI platform and engaged member base. You will serve as the physician leader and architect of this new program, responsible for designing the entire care model, building the clinical team, implementing AI-enabled workflows, and delivering direct patient care. Key responsibilities include: **Care Model Design**: Define the clinical architecture end-to-end—which MSK conditions to address, member triage and intake processes, visit structures, escalation pathways for imaging and surgery, pain management strategies (with opioid stewardship), and peri-operative optimization. You will establish protocols for low back and neck pain, hip and knee osteoarthritis, shoulder pain, fragility fractures, osteoporosis, falls, and deconditioning with enough precision to scale nationally. **AI System Development**: Partner with product and engineering teams to build the agentic interface that delivers care. You will encode clinical reasoning, specify system behavior and escalation rules, shape member experience between visits, and supervise the AI system as you would a clinical fellow. This includes writing prompts, reviewing transcripts, and iterating on edge cases. **Clinical Delivery**: Carry a shared patient panel and practice within the model you design. You will serve as the clinical backstop for diagnostic ambiguity, red flags, complex multimorbid cases, and situations where standard pathways don't apply. **Team Building and Leadership**: Hire, train, and lead Advanced Practice Providers, physical therapists, and care team members across multiple states. You will set standards for clinical encounters, establish collaborative practice agreements, run case reviews and interdisciplinary rounds, and grow a nationally scalable team while maintaining trust and empathy. **Quality and Measurement**: Build quality measures, balanced scorecards, and feedback loops focused on function and pain outcomes, appropriate imaging and surgical referral rates, opioid stewardship, and member experience. Use these to rapidly iterate the care model and AI system. This is a player-coach role where you will practice the medicine you design, and your model will shape how tens of thousands of seniors experience MSK care. You will work with an existing infrastructure of virtual and in-home clinical programs and an engaged member base. **Requirements**: - Board certification in Physical Medicine and Rehabilitation, Sports Medicine, Rheumatology, Orthopedic Surgery, or Internal Medicine/Family Medicine with substantial MSK practice - Significant clinical experience managing common MSK conditions in adults, including older adults with multimorbidity - Demonstrated experience building or substantially redesigning a clinical program, pathway, or service line (not just running an existing one) - Deep commitment to measuring and improving quality across a population - Experience serving diverse patient populations aligned with treating every member like family **Desired Experience**: - 5+ years delivering MSK care in sports medicine, PM&R, or orthopedics practice - Experience supervising Advanced Practice Providers, physical therapists, or other clinicians - Direct experience working with product and engineering teams on clinical software or AI-enabled workflows - Virtual or hybrid care delivery experience - Familiarity with Medicare Advantage populations and value-based care economics - Experience with peri-operative optimization, post-acute recovery, or falls and fracture prevention programs
