Habitat Health is seeking a Medical Director to lead clinical operations at a PACE (Program of All-Inclusive Care for the Elderly) center in Chicago. This is a senior clinical leadership role accountable for quality, safety, participant experience, and outcomes of care delivered to older adults.
The Medical Director serves as a dyad leader alongside the Executive Director, balancing direct clinical care, clinical leadership, team development, participant experience, operational reliability, and fiscal stewardship. The role combines hands-on geriatric medicine with organizational leadership.
Key responsibilities include:
**Direct Clinical Care & Panel Management**: Provide evidence-based, person-centered primary care to approximately 50% clinical panel (subject to census and operational need). Supervise other physicians and collaborate with the interdisciplinary team on person-centered care planning across medical, behavioral, and functional domains. Apply strong clinical judgment in evaluation, diagnosis, and management with geriatric principles including preventive care and harm reduction. Lead safe medication management, deprescribing, and polypharmacy reduction. Produce high-quality documentation supporting continuity, coding integrity, and care coordination.
**Clinical Leadership & Team Development**: Oversee clinical quality, safety protocols, and regulatory compliance. Mentor and develop clinical staff. Lead quality improvement initiatives and evidence-based practice adoption. Ensure adherence to PACE model principles and clinical standards.
**Participant Experience & Outcomes**: Champion person-centered care delivery. Monitor and improve participant health outcomes, satisfaction, and functional status. Lead care planning that honors participant goals and preferences.
**Operational & Fiscal Stewardship**: Partner with Executive Director on center performance, budget management, and operational efficiency. Support sustainable, scalable care delivery aligned with Habitat Health's mission.
