The company operates owned clinics (Centrum Health and Premier Medical) and partners with affiliated providers nationwide, using technology and services to enable providers to succeed in performance-based arrangements. As Manager of Care Management, you will directly supervise and manage clinical staff supporting NeueHealth's care management programs. You'll provide day-to-day supervision of care management teams, ensuring consistent application of clinical standards and proper documentation. Your primary responsibility is aligning clinical teams with NeueHealth's Model of Care. Key responsibilities include: performing clinical functions within your scope of practice; monitoring performance and outcome data at defined intervals to track program activities and key performance indicators; supporting the clinical director in developing performance improvement plans based on KPI deficiencies; leading onboarding training and continued education for clinical staff and care partners; ensuring consistent application of the care model across clinical services; completing value stream mapping and desktop procedures for program standardization; supporting clinical directors in conducting care team meetings; and adhering to Quality Management Committee policies. Required qualifications: Active, unrestricted California RN license; bachelor's degree in nursing, business, or related healthcare field (preferred); minimum 5 years' experience in medical management (case management, utilization management); minimum 1 year supervisory experience; certified case management (preferred); training experience (preferred); and EZ Care proficiency. You must demonstrate high attention to detail, strong analytical skills, critical thinking, and problem-solving abilities.
