Stellar Health is a healthtech platform that helps primary care providers transition from fee-for-service to value-based care models. The company combines technology, analytics, and people to support providers at the point of care, delivering real-time patient information and incentivizing quality outcomes. Stellar is backed by top-tier VCs including General Atlantic, Point72, and Primary Venture Partners.
The Lead, ACO Performance role is a general manager position responsible for two of Stellar's most important Medicare ACO relationships: participation in CMS's LEAD (Long-term Enhanced ACO Design) Model and a partnership with an academic health system's affiliated provider network. You will own end-to-end performance accountability for quality, total cost of care, and shared savings/risk results across both programs.
Key responsibilities include:
- Serve as general manager of two CMS ACO accounts with full accountability for aggregate performance metrics
- Define benchmarks and success criteria for each ACO in partnership with Analytics and Medical Economics teams
- Own governance and executive reporting, including steering committee agendas, materials, and facilitation
- Analyze quality, claims, and cost data to identify performance drivers and translate findings into actionable recommendations
- Partner with Provider Success teams to ensure performance gaps have data-driven action plans and track execution
- Recommend and refine SVU menus and incentive designs to optimize outcomes for Stellar, ACOs, and providers
- Stay current on LEAD Model program requirements, benchmarking milestones, and reporting obligations
- Proactively identify risks to ACO performance and escalate to VP, Nova Performance with clear mitigation plans
- Contribute best practices and lessons learned across the Nova Performance team to build scalable playbooks
You will work largely autonomously, leading executive-facing meetings and communicating performance and risk to both internal leadership and external health system partners. The role requires strong analytical capabilities, Excel and BI tool fluency, and the ability to turn performance data into clear insights with explicit business implications.
REQUIREMENTS:
- 4+ years of experience in healthcare operations, value-based care, ACO/managed care, or account management within a health-tech or provider services environment
- Knowledge of Medicare total cost of care risk models (MSSP, ACO REACH, LEAD Model, or similar) strongly preferred
- Track record of owning a book of business or set of accounts end-to-end, operating autonomously and delivering measurable outcomes with limited day-to-day oversight
- Comfort leading executive-facing meetings (steering committees, operating reviews) and communicating performance and risk clearly to internal and external stakeholders
- Strong analytical skills including Excel and BI tool fluency; ability to turn performance data into clear insight and actionable "so what"
- Ability to work independently against ambiguous problems, set own priorities, and proactively flag when longer-term direction input is needed
- Excellent written and verbal communication skills with ability to build trust quickly across cross-functional teams and external stakeholders
- Collaborative mindset; comfortable driving outcomes through influence and partnership rather than direct authority alone
