The Orchestrator product addresses a critical gap in healthcare: specialist referrals that fall apart through manual handoffs, faxes, and lost clinical context. Orchestrator ingests referrals from multiple sources (fax, phone, direct EHR feed), extracts clinical and demographic data, automates coordinator workflows, and writes results back into the EHR. It combines document AI, voice automation, and healthcare integration engineering.
You will lead an existing team with a shipping product and real customers, growing it meaningfully over the next year. You will own end-to-end delivery of the Orchestrator platform, including the intake pipeline, coordinator-facing application, voice automation, and EHR write-back. You will drive architecture decisions, run hiring and onboarding, and work closely with the Director of Engineering on roadmap and prioritization.
Key responsibilities include: owning delivery end-to-end; growing the team through hiring and raising technical standards; staying deeply technical (design docs, code review, production incidents); setting integration strategy for healthcare standards (HL7 v2, FHIR, X12) versus UI automation; scaling the system for growing referral volume and customer count; partnering directly with customers on complex workflows; translating customer needs into generalized product abstractions; and coaching engineers into larger scope.
This role is ideal if you have 1-2 years of management experience, a strong IC engineering background, and want scope to grow quickly. You will work directly alongside clinicians and see immediate impact on patient care.
REQUIREMENTS:
Required:
- Direct experience building software that integrates with EHRs. Must have shipped and supported an integration against at least one major EHR platform and can speak concretely about implementation challenges.
- Working fluency in healthcare interoperability: HL7 v2 message structure, FHIR resources, and operational reality of interface engines and site-specific customization.
- Strong full-stack engineering background. Must have built and operated production services and interfaces, with opinions on both.
- At least 1 year managing engineers, on top of 5+ years as an IC engineer. Depth of IC foundation valued more than length of management track record.
- Comfort with ambiguity in the problem space. Requirements come from observing workflows, not specs.
- Track record of building products serving many customers. Must have experience saying no to one-off requests or managing the consequences of saying yes.
Nice to have:
- Experience with document AI or OCR pipelines at scale.
- Experience with voice agents or telephony infrastructure (SIP, IVR navigation, real-time speech).
- Background in referral management, prior authorization, care coordination, or revenue cycle.
- Experience selling and supporting into large health systems, including security review and integration onboarding.
