Habitat Health is a venture-backed healthtech company transforming care for older adults through the Program of All-Inclusive Care for the Elderly (PACE). The company delivers comprehensive medical care alongside support for daily needs—meals, transportation, in-home assistance—in a fully integrated model. Supported by NEA, Kaiser Permanente, and Town Hall Ventures, Habitat is in a period of significant expansion.
The Director of Partnerships is a strategic, cross-functional leadership role central to Habitat's operating model and growth thesis. This leader will manage partner relationships across all organizational levels, from day-to-day operations teams to senior executives. Key responsibilities include developing and executing partner-facing workflows and tools to improve care delivery and market performance; solving complex operational and partner-facing issues at a systems level; building a scalable partnership management structure that supports new center launches across diverse partner archetypes and markets; and adapting Habitat's operations to support expansion into new geographic markets while respecting local partner needs.
The role requires delivering executive updates to both Habitat and partner leadership, connecting internal initiatives to external commitments, and supporting broader strategic enterprise initiatives where they intersect with partnerships. This is a high-impact position requiring strong communication, sound judgment in ambiguous situations, and a proven track record managing cross-cutting priorities and complex organizational dynamics.
Candidates must reside in California (Bay Area or Greater Los Angeles preferred) and be willing to travel up to 25%.
