Wider Circle is seeking a Community Engagement Specialist to lead the Connect for Life program in Philadelphia, a groundbreaking initiative that connects neighbors to improve health outcomes and combat healthcare inequity. This role is central to building trusted relationships within communities and empowering members to live longer, healthier lives. Key responsibilities include leading and hosting virtual and in-person events that foster relationships among program members, strategically planning community activities including venue selection and member outreach to confirm attendance. You will proactively engage members through multiple channels—group events, one-on-one home visits, and telephone outreach—to ensure consistent participation and address care needs. You will maintain detailed documentation of all member interactions, attendance, and outreach attempts using Wider Circle's digital systems. A critical part of the role involves identifying natural community leaders and recruiting them as Wider Circle Ambassadors to enhance member connections and expand community reach. You will also conduct prospective member outreach, developing personalized enrollment plans via phone, in-person meetings, or events. Additionally, you will serve as a liaison connecting members to health plans and healthcare services, helping them navigate doctor appointments and close care gaps. You are accountable for achieving membership engagement and growth goals, tracking success through attendance metrics, member feedback, and relevant data. The role requires regular in-home visits, safe driving up to 30 miles from home, carrying supplies up to 30 pounds, and the ability to speak publicly and engage effectively both in-person and virtually. You must be comfortable with computer systems for data entry and tracking, and able to communicate clearly with diverse community members.
