Salary: USD 50,000 - 95,000 / annual
Ennoble Care is a mobile primary care, palliative care, and hospice service provider operating across 11 states (New York, New Jersey, Maryland, DC, Virginia, Oklahoma, Kansas, Pennsylvania, Texas, Florida, and Georgia). The company brings clinical care directly to patients' homes, focusing on those with chronic conditions and limited mobility, offering remote patient monitoring, behavioral health management, and chronic care management.
The Clinical Liaison serves as the first point of contact for hospitalized patients and families who may benefit from Ennoble Care's services. This role bridges the gap between acute care facilities and Ennoble Care's clinical teams, educating patients and families about hospice and palliative care options, and facilitating smooth transitions of care.
Key responsibilities include:
- Identifying and liaising with hospitalized patients appropriate for hospice or palliative care services
- Educating patients, families, and healthcare partners about Ennoble Care's programs and philosophy
- Providing emotional support and guidance during care transitions
- Conducting nursing assessments and making clinical judgments based on patient needs and eligibility
- Collaborating with physicians, interdisciplinary teams, and acute care partners to develop individualized, goal-aligned care plans
- Initiating care plans and coordinating discharge planning with medical consultants
- Communicating patient goals, clinical status, and treatment plans with team members
- Promoting Ennoble Care's programs within hospitals, skilled nursing facilities, and other referral sources
- Building relationships with facility leadership through education, in-service trainings, and community outreach
- Rotating to other regions or departments as business needs require
Requirements:
- Minimum 1 year of experience in a comparable clinical setting or unit
- Minimum 1 year of hospice and/or palliative care experience
- Registered Nurse (RN) or Licensed Practical Nurse (LPN) licensure required; graduate of accredited nursing program
- BSN preferred for RN candidates
- Current nursing licensure as RN or LPN in applicable state(s); compact state licensure accepted
- Valid driver's license in good standing with access to reliable vehicle during working hours
