Salary: USD 200,000 - 240,000 / annual

Pine Park Health is a value-based primary care practice transforming healthcare for seniors in senior living communities across Arizona, California, and Nevada. The company brings healthcare directly to residents' homes, offering prevention, screening, chronic condition management, lab work, and diagnostic testing. With weekly community visits and same/next-day urgent care appointments, Pine Park eliminates barriers to care and helps seniors avoid unnecessary emergency room visits and hospitalizations. You will serve as Clinical Director for the Bay Area region, leading and developing a small team of Nurse Practitioners and Physician Assistants while maintaining hands-on patient care. This is a hybrid leadership role that balances people management with clinical practice. **Key Responsibilities:** - Lead, coach, and develop a small team of NPs and PAs across the Bay Area - Spend regular time in the field alongside providers—observe care, coach in real time, and stay connected to day-to-day work - Conduct 1:1s and provide ongoing feedback and professional development - Coach providers toward strong clinical and operational performance using metrics such as quality, documentation, access, and care-gap closure - Drive hiring and team growth - Partner with community leadership and facility staff to address comprehensive health needs - Contribute to clinical protocols, quality improvement, and operational excellence - Maintain your own patient panel while managing leadership responsibilities **Work Arrangement:** This role is based in the Bay Area with flexibility to plan on-site time around provider and patient needs. You'll receive mileage reimbursement and have Fridays designated for work-from-home learning and development to maintain sustainability. **Requirements:** - Active California Physician Assistant license - Active unrestricted DEA registration - Master's degree in Physician Assistant studies - Meaningful independent clinical practice experience - Hands-on people leadership experience (training, coaching, performance management, regular 1:1s) with clinical or non-clinical staff. Lead NP, APP Manager, Clinical Manager, or Clinical Supervisor roles preferred; Director title not required - Strong clinical judgment; able to assess, care-plan, and manage chronic conditions independently - Strong communication and relationship-building skills for coaching teammates, partnering with community teams, and navigating difficult conversations - Comfort working autonomously in a fast-paced, multi-site environment **Preferred:** - Primary care, geriatrics, internal medicine, complex/chronic care, home-based or senior care, population health, or value-based care experience - Family or Adult-Gerontology specialization
