As Care Team Manager for the Grand Rapids market, you will lead daily clinical operations and build a high-performing team of Registered Nurses, Nurse Practitioners, LICSWs, Health Partners, and administrative staff. You'll manage team members, oversee schedules, develop policies and procedures, and ensure smooth care delivery across virtual, in-person, and at-home settings.
Key responsibilities include:
**Clinical Operations Leadership**: Lead day-to-day operations including staffing, scheduling, and adherence to clinical standard operating procedures. Build and sustain a cohesive team culture delivering outstanding patient outcomes. Hire, onboard, coach, and develop clinical and administrative staff. Partner with the Market Operations Leader and Medical Director on strategy and operations supporting market growth.
**Data-Driven Execution**: Monitor performance using operational and clinical data to identify improvement opportunities and drive timely action. Set clear performance metrics across quality, patient experience, productivity, and efficiency. Foster continuous improvement culture using data to spot trends, solve problems, and measure results.
**Care Delivery Oversight**: Ensure care delivery across virtual, in-person, and at-home settings is efficient and high quality. Implement strategies improving access and quality while optimizing Care Team time. Handle escalated patient issues and drive service recovery.
**Partnerships & Market Development**: Serve as trusted partner to Grand Rapids providers and market stakeholders, supporting Daymark's visibility and growth.
**Cross-Functional Collaboration**: Work with Enrollment, Clinical Operations, Tech, Marketing, and Growth teams to align Care Team efforts with organizational goals.
You bring 5+ years of healthcare operations, care management, or clinical leadership experience with prior people management. Strong background in patient care delivery and clinical operations preferred, with experience in virtual care and/or at-home healthcare models a plus. Clinical background (RN, NP, LICSW) is advantageous. You excel at hiring, onboarding, and developing clinical and administrative staff in fast-paced environments. You're deeply committed to exceptional quality of care, understand complex care models, and possess strong operational acumen in scheduling, staffing, and budget management. Data-driven mindset with ability to monitor clinical outcomes and track performance metrics. Excellent interpersonal and communication skills building trust with clinical staff, community partners, patients, and families. Comfortable with ambiguity and startup environments, able to build and iterate on processes while maintaining high care standards.
