Bluestone Physician Services is seeking a Care Coordinator with a Licensed Social Worker (LSW) credential to join their team serving seniors in Minnesota. This hybrid role combines remote work with regular field visits (50-70% travel, 30-50% remote) throughout the Blaine area and surrounding communities.
You will manage member caseloads within your assigned geographic area, coordinating care for seniors enrolled in Minnesota Senior Health Options (MSHO/MSC+) programs. Key responsibilities include conducting face-to-face visits, managing Elderly Waiver programs, performing annual assessments including Personal Care Assistance (PCA) evaluations, developing individualized care plans, and connecting members with community and state resources. You'll collaborate with multidisciplinary care teams to ensure health care quality measures are met, support members during care transitions, and complete required documentation.

The position offers a full-time schedule with day shift hours (8am-5pm Mon-Thu, 8am-3pm Fri), no evenings, weekends, or holidays. You'll work from home but travel regularly to serve members in assisted living facilities and community settings. Required qualifications: Current Minnesota Licensed Social Worker license (new graduates encouraged) or Minnesota Registered Nurse license, valid driver's license, and preferred experience (1+ years) in geriatric or disabled population case management, home care, group homes, nursing homes, or assisted living. You must have access to a private, HIPAA-compliant home office space and demonstrate strong independent work capability, problem-solving skills, and excellent communication abilities.
