Salary: USD 58,000 - 82,000 / annual

Devoted Health is seeking a Supervisor for Transitions of Care to lead a team of Associates supporting patients during critical moments—transitioning home after hospital stays. This is a hybrid people-management and hands-on clinical role: approximately 70% supervisory (team leadership, coaching, performance management, KPI monitoring) and 30% direct patient care delivery, with clinical time fluctuating based on team size. You will lead a team of Transitions of Care Associates, conducting regular 1:1s, team meetings, and coaching sessions to drive performance, quality, and patient experience. You'll create a supportive, inclusive, high-performing culture grounded in Devoted's member-centered values, manage onboarding and competency development, and ensure adherence to TOC workflows and documentation standards. Key responsibilities include monitoring daily/weekly/monthly KPIs, identifying trends and improvement opportunities, partnering with leadership on staffing and scheduling, and guiding associates through complex cases. The ideal candidate brings an active Medical Assistant Certification, 2+ years of direct lead or supervisory experience in a virtual setting, and a mission-driven, proactive approach to developing people and improving processes. Desired qualifications include experience in remote, high-volume, fast-paced environments; transitional care or post-acute workflows; strong organizational skills and attention to detail; and bilingual English/Spanish fluency (strongly preferred).
