WelbeHealth PACE is a mission-driven healthcare organization dedicated to helping seniors remain in their homes and communities by providing comprehensive medical care and community-based services. The Social Work Manager leads the Social Work team and oversees the effectiveness of social services programs while serving as a member of the leadership team.
In this role, you will manage Social Workers and Community Health Workers, including hiring, training, supervising, and coaching. You will oversee the effectiveness of Community Health Workers and interdisciplinary team (IDT) participation, ensuring outstanding delivery of all social work interventions aligned with participant care plans and urgent needs. Key responsibilities include ensuring compliance across all social work areas—documentation, assessment, care plan completion and review, and annual review of participant rights and grievance processes.
You will oversee programming in collaboration with the Behavioral Health Specialist, including support groups, individual counseling, and family education, partnering with community professionals as needed. Additional duties include participating in end-of-life care coordination and support, conducting home visits as necessary, and establishing deep relationships with participants on your caseload. The role emphasizes collaborative relationships across WelbeHealth and working as an integral member of an interdisciplinary team rather than in isolation.
WelbeHealth offers a competitive total rewards package including medical, dental, and vision insurance; 17 days of PTO, 12 observed holidays, and 6 sick days annually; 401(k) savings with match; and a comprehensive compensation package with base pay and bonus.
