Habitat Health is a rapidly growing healthcare organization delivering comprehensive care to older adults through the Program of All-Inclusive Care for the Elderly (PACE) model. You will own the product strategy, roadmap, and cross-functional delivery for Habitat Health's technology application portfolio as a Product Manager reporting directly to the Vice President of Product. This role sits within the Strategic Technology Applications team, which functions as the internal product organization for systems powering clinical, operational, growth, and administrative workflows. Key responsibilities include: **Product Strategy & Roadmap**: Own the vision and strategy for a portfolio of strategic applications including call/communications platforms (TalkDesk), CRM (Salesforce), and EHR (Epic). Translate clinical, operational, and growth stakeholder needs into a prioritized roadmap. Define and track success metrics around adoption, utilization, satisfaction, and operational impact. Maintain visibility into the application portfolio's health, dependencies, and integration points. **Discovery, Delivery & Execution**: Conduct discovery with stakeholders to surface pain points and validate requirements. Partner with internal analysts and external vendors to scope, prioritize, and deliver configuration changes, integrations, and features. Own backlog grooming, sprint planning, and prioritization using Jira. Lead technology application implementations for new PACE center launches, including configuration, workflow validation, UAT, and go-live support. Build repeatable implementation and product-launch playbooks. **Vendor & Stakeholder Management**: Manage day-to-day vendor relationships for managed applications, including performance management, escalations, and contract oversight. Evaluate and recommend new technology solutions and support build-vs-buy assessments. Serve as a trusted product partner for clinical, operational, and growth teams. Represent the portfolio in cross-functional forums and governance bodies. **Adoption & Training**: Drive user adoption through training programs, change management, and feedback loops. Monitor utilization metrics and iterate based on user feedback. The ideal candidate is a product-minded healthcare technology leader energized by translating clinical and operational needs into compelling roadmaps, partners naturally with internal developers and vendors, and uses data and stakeholder feedback to drive continuous improvement. The role requires regular travel for onsite go-lives, department rounding, and support activities.
