Salary: USD 220,000 - 240,000 / annual

Twin Health is a digital health company applying AI Digital Twin technology to help people prevent and manage chronic metabolic diseases like type 2 diabetes and obesity. The company builds dynamic metabolic models for each person using data from continuous glucose monitors, smartwatches, and meal logs, paired with dedicated clinical care teams. As Principal Product Designer, you will own the end-to-end experience strategy for Twin's mobile app—the primary touchpoint where members manage their health daily. This role focuses on inventing interaction patterns for agentic AI in healthcare from scratch. There is no established playbook; agentic experiences where AI acts autonomously, surfaces proactive guidance, and adapts in real time to behavior and health data represent one of the least explored frontiers in product design. You will be the person who defines what these patterns look, feel, and behave like for people managing chronic conditions. This requires genuine originality, comfort with ambiguity, and creative confidence to propose entirely new solutions. You will set the quality standard others aspire to, shape product roadmaps beyond your immediate scope, and leave a design legacy that outlasts individual features. Key responsibilities include: identifying and driving mission-critical design initiatives with significant scope and long-term timelines; pioneering agentic UX from 0 to 1 by designing entirely new interaction patterns for AI systems that act proactively and with increasing autonomy; assisting stakeholders across teams in shaping product roadmaps from a design-led perspective; leading design strategy across interconnected member journeys from onboarding through long-term behavior change; inventing in uncharted territory with high creative originality; leading high-complexity initiatives end-to-end while serving as a design consultant on smaller efforts; driving innovation workshops and ideation exercises; setting the quality bar for design at Twin across interaction design, visual design, copy direction, and systems thinking; translating clinical complexity and AI-generated insights into intuitive and motivating experiences; owning full craft end-to-end from information architecture through pixel-perfect specs; identifying and influencing resolution of visual, accessibility, and interaction issues; stewarding and building Twin's design system with new agentic design patterns as reusable components; mentoring designers and cross-functional partners; developing relationships with senior stakeholders and executives; defining and running research that uncovers unmet needs and validates direction; communicating with expert fluency to clinicians, engineers, and executives; and making contributions that exceed expected boundaries of a product design role. Requirements: - 10+ years of product/UX design experience with a portfolio demonstrating highest level of craft across interaction design, visual design, and systems thinking - Demonstrated experience designing agentic AI experiences where AI takes action, surfaces proactive interventions, or operates with agency - Proven ability to design 0-to-1 and create original interaction patterns in spaces with no prior art - Highly inventive and creatively courageous; track record of work that others in the industry reference - Recognized thought leader in design internally and externally whose perspective carries weight beyond immediate team - Expert communication and storytelling skills; able to negotiate with and persuade leaders at all levels with humility and without ego - High degree of independence and ingenuity solving critical design challenges with track record of identifying and executing new opportunities without waiting to be directed - Bachelor's degree in Design, HCI, or related field, or equivalent experience - Must be authorized to work for any employer in the U.S.; unable to sponsor or take over sponsorship of employment visa Nice to have: - Hands-on experience with LLM prompting as part of design or prototyping workflow - Experience establishing design patterns for AI transparency, explainability, or human-in-the-loop interactions
