Salary: USD 150,009.6 - 190,216 / annual

WelbeHealth PACE (Program of All-Inclusive Care for the Elderly) is seeking a Primary Care Provider (Nurse Practitioner or Physician Assistant) to deliver comprehensive medical care to seniors in their homes and communities. Working under the direction of the Medical Director, you will collaborate with physicians and an interdisciplinary team (IDT) to complete assessments, develop care plans, and educate participants and families. Key responsibilities include: - Provide continuous medical coverage and perform comprehensive in-person assessments (initial, semi-annual, and unscheduled) for participants - Collaborate with physicians and IDT members to develop participant care plans, oversee care coordination, and arrange discharge planning from acute or long-term settings - Maintain participant care in skilled nursing facilities and residences with regular visits based on participant need - Review outcomes and utilization data, including hospitalizations and specialist use, to establish best practices and policy improvements What makes this role distinctive: You'll serve a smaller patient caseload, enabling long-term relationships with participants and true value-based care. Requirements: - Graduation from an accredited school of nursing - Current Nurse Practitioner or Physician Assistant certification with prescriptive authority - Current RN state license - Minimum 2 years of experience working with frail or elderly populations - Current knowledge of chronic care, geriatric issues, and best practices - Reliable transportation - Ability and willingness to participate in rotating night and weekend call - Preferred: experience caring for seniors with complex care needs (Medicare/Medicaid population)
