Aledade is seeking a Practice Transformation Specialist (PTS) to serve as a direct interface between the company's headquarters and healthcare provider partners. In this role, you will be the primary point of contact for medical practices, gathering insights into partner satisfaction, operational needs, and care delivery challenges. You will work closely with practices to implement population health strategies, optimize clinical workflows, and identify opportunities to improve patient outcomes while reducing costs.
As a PTS, you will combine deep knowledge of healthcare operations with consultative problem-solving skills. You'll conduct practice assessments, develop transformation roadmaps, and guide implementation of evidence-based care models. Your work will involve regular on-site visits to partner practices, stakeholder interviews, data analysis, and coordination with Aledade's clinical and technical teams to ensure successful adoption of new processes and technologies.
The ideal candidate is passionate about population health and mission-driven to improve healthcare delivery. You should have strong communication skills, the ability to build trust with clinical and administrative leaders, and a track record of driving organizational change. Experience in healthcare consulting, practice management, or health system operations is highly valued. You will be based in Indianapolis, IN, serving the Midwest region.
