Ennoble Care is a mobile primary care, palliative care, and hospice service provider operating across 11 states (NY, NJ, MD, DC, VA, OK, KS, PA, TX, FL, GA). The company brings clinicians to patients' homes and long-term care facilities, emphasizing continuity of care and relationship-building for those with chronic conditions and limited mobility. You will serve as an Embedded Care Coordinator, working full-time on-site within one or more partnered long-term care facilities in the Atlanta/Decatur area. This is a non-remote, in-person role where you become an integral member of the clinical care team, working alongside Ennoble Care providers in a family-practice-style model. Key responsibilities include: - Coordinate care for long-term care residents using Ennoble Care's EMR system - Maintain regular, proactive on-site engagement with residents to support care plans, follow-ups, and care transitions - Collaborate closely with Ennoble Care providers and facility staff - Serve as a key liaison between providers, facility staff, residents, and families - Communicate patient needs, changes in condition, and urgent concerns to the clinical team in a timely manner - Accurately document patient interactions and care coordination activities - Follow established workflows, documentation standards, and escalation protocols - Contribute positively to team collaboration and strong facility partnerships The "Embedded" designation means you are physically present in the facility daily, building consistent relationships with residents and families, and becoming a trusted, familiar presence. The work emphasizes remote patient monitoring (RPM) and proactive care coordination in a family-practice-focused environment. Schedule: Monday–Friday, 8:30 a.m. to 5:00 p.m. EST (or per facility hours). No weekends, no on-call, daytime hours only. Seven company-paid holidays annually. Requirements: - Legally authorized to work in the United States - Active, unrestricted clinical certification or licensure: Patient Care Technician (PCT), Certified Nursing Assistant (CNA), Medical Assistant (MA) / Certified Medical Assistant (CMA), Licensed Practical Nurse (LPN), Registered Nurse (RN), or comparable credential - Minimum 1 year of patient-facing experience in a long-term care, skilled nursing, or similar clinical setting - Comfortable communicating proactively and professionally via Microsoft Teams, email, phone, and in person - Ability to cultivate and maintain positive, approachable relationships with residents, families, providers, and facility staff - Well-developed organizational and time-management skills with an optimistic, initiative-driven mindset - Strong working knowledge of healthcare terminology and care coordination principles - Proficiency with Microsoft 365 (Outlook, Word, Excel) - Valid, unencumbered driver's license with consistent, reliable transportation - Current auto insurance with proof available - Ability to pass background check and drug screen - Travel between facilities may be required
