Aledade is seeking a Practice Transformation Specialist (PTS) to serve as the primary liaison between headquarters and healthcare provider practices in South Carolina. This role is central to Aledade's mission of improving patient care and reducing costs through population health initiatives.
As a PTS, you will be responsible for driving transformation initiatives within partner practices, serving as the direct interface between Aledade's leadership and on-the-ground providers. Your core responsibilities include understanding practice needs, identifying opportunities for operational improvement, and implementing strategies that enhance patient outcomes while optimizing cost structures.
The ideal candidate is deeply passionate about population health and mission-driven to deliver better care to patients. You will work closely with practice leadership and clinical teams to assess current workflows, identify gaps, and guide the adoption of Aledade's tools and methodologies. Your insights will be invaluable in shaping how Aledade supports its partner network.
This position requires a strong understanding of healthcare operations, excellent communication skills, and the ability to build trust with diverse stakeholder groups including physicians, practice administrators, and clinical staff. You will need to be comfortable with data analysis, process improvement methodologies, and change management.
