Bluestone Physician Services is seeking a Care Coordinator with a Licensed Social Worker (LSW) credential to join their multidisciplinary care team serving seniors and disabled populations across Minnesota, Wisconsin, and Florida. This hybrid role is based in the Woodbury area and focuses on managing caseloads of MSHO/MSC+ members enrolled through the Department of Human Services and Centers for Medicare & Medicaid Services.
In this position, you will coordinate comprehensive care for complex, chronic conditions by conducting face-to-face member visits (50–70% of time), managing the Elderly Waiver program, and performing annual assessments including Personal Care Assistance (PCA) evaluations. You'll develop individualized care plans, identify and connect members with community and state resources, and collaborate with medical care teams to ensure quality outcomes and value-based care goals are met. Key responsibilities include managing transitions of care, supporting safe discharges, completing required documentation, and helping members remain safely in their homes and communities.
The schedule is full-time with Monday–Thursday 8 AM–5 PM and Friday 8 AM–3 PM, with no evenings, weekends, holidays, or on-call requirements. Required qualifications include a current Minnesota Licensed Social Worker credential (LSW, LMSW, LISW, or LICSW) or Minnesota Registered Nurse license. One or more years of experience working with geriatric or certified disabled populations in case management, care coordination, home care, group homes, nursing homes, or assisted living settings is preferred. New graduates with LSW credentials are encouraged to apply.
