Ennoble Care is a mobile primary care, palliative care, and hospice service provider operating across 11 states. This role is a full-time, in-person Care Coordinator embedded within one or more partnered long-term care facilities in the Atlanta area, with primary focus on Sandy Springs. You will serve as an integral member of the clinical care team, working alongside Ennoble Care providers in a family-practice-style model emphasizing continuity, relationship-building, and patient-centered support. This is not a remote or call-center role—you are physically on-site daily, building consistent relationships with residents, families, and facility staff. Key Responsibilities: - Coordinate care for long-term care residents using Ennoble Care's EMR system - Maintain regular, proactive on-site engagement with residents to support care plans, follow-ups, and care transitions - Collaborate closely with Ennoble Care providers within the long-term care setting - Serve as a key liaison between providers, facility staff, residents, and families - Communicate patient needs, changes in condition, and urgent concerns to the clinical team in a timely manner - Accurately document patient interactions and care coordination activities - Follow established workflows, documentation standards, and escalation protocols - Contribute positively to team collaboration and strong facility partnerships - Support remote patient monitoring (RPM) and proactive care coordination initiatives Work Schedule: - Monday through Friday, 8:30 a.m. to 5:00 p.m. EST (or thereabouts, contingent on facility hours) - No weekends, no on-call, daytime hours only - Seven company-paid holidays annually Requirements: - Legally authorized to work in the United States - Active, unrestricted clinical certification or licensure: Patient Care Technician (PCT), Certified Nursing Assistant (CNA), Medical Assistant (MA) / Certified Medical Assistant (CMA), Licensed Practical Nurse (LPN), Registered Nurse (RN), or comparable credential - Minimum 1 year of patient-facing experience in a long-term care, skilled nursing, or similar clinical setting - Comfortable communicating proactively and professionally via Microsoft Teams, email, phone, and in person - Ability to cultivate and maintain positive, approachable relationships with residents, families, providers, and facility staff - Well-developed organizational and time-management skills with an optimistic, initiative-driven mindset - Strong working knowledge of healthcare terminology and care coordination principles - Proficiency with Microsoft 365 (Outlook, Word, Excel) - Valid, unencumbered driver's license with consistent, reliable transportation - Current auto insurance with ability to provide proof - Ability to pass background check and drug screen - Travel between facilities may be required
