As an Advanced Practice Provider, you will deliver comprehensive primary care to a dedicated patient panel in Brevard, NC. The role emphasizes meaningful patient relationships and clinical autonomy within a supportive team environment. You'll work with longer appointment times, dedicated preparation and charting time, and a manageable patient panel of approximately 15 patients per day. Administrative burden is reduced through dedicated Medical Assistants and Clinical Documentation Specialists who support clinical workflows.
Key responsibilities include:
- Providing comprehensive primary care through in-person and telehealth visits
- Managing routine, acute, and chronic medical conditions
- Completing comprehensive Annual Wellness Visits and Health Risk Assessments
- Collaborating with physicians and interdisciplinary team members (RN care managers, community health workers) to coordinate whole-person care
- Using evidence-based guidelines and clinical best practices
- Identifying opportunities to improve care delivery and patient outcomes
- Leveraging data and quality metrics (STARS, HEDIS, utilization, patient experience) to drive continuous improvement
- Contributing to innovative solutions for challenges facing rural patients

REQUIREMENTS:
- 2+ years of experience practicing in primary care (Internal Medicine, Family Medicine, or Geriatrics)
- Experience with HCC documentation, ICD-10 coding, and Health Risk Assessments (HRAs)
- Comfort working across multiple technology platforms, including electronic health records and spreadsheets
- Strong organizational and time-management skills with ability to manage multiple priorities
- Continuous-improvement mindset and enthusiasm for using data to improve care
- Excellent communication and collaboration skills
- Active, non-probationary state medical license
- Authorization to work in the United States
