Sollis Health is seeking an Advanced Care Provider (Physician Assistant or Nurse Practitioner) to join its at-home concierge house calls team in New York. This is a full-time position working 9am–9pm shifts, 3 days per week, providing urgent and emergent medical care to adult and pediatric patients in their homes, reducing unnecessary emergency department visits.
In this role, you will conduct comprehensive medical evaluations and formulate differential diagnoses and treatment plans for patients in home and non-clinic settings. You will perform a range of clinical procedures including IV access and medication administration, phlebotomy, splinting, suturing, point-of-care testing, and EKGs. You will manage COVID-related house calls with full PPE protocols, including clinical sampling and antibody testing. You will coordinate with House Call Coordinators to respond timely to patient requests, maintain provider bags and supplies, document care in eClinicalWorks EHR, and interface with patients' primary care physicians and specialists to coordinate follow-up care.
Additional responsibilities include working closely with the Medical Director and clinical team to ensure policy adherence and quality care delivery, coordinating handoffs of pending labs and imaging to in-clinic providers at shift end, and occasionally covering shifts at Sollis' flagship centers and clinics as needed. You will provide patient-centered, empathetic communication and be comfortable serving high-net-worth clientele.
Sollis Health operates a 24/7, 365-day on-demand medical membership model with locations in Manhattan, the Hamptons, Los Angeles, South Florida, and San Francisco. Members have unlimited access to ER-trained medical teams, on-site labs and imaging, expedited specialist appointments, and integrated care navigation.
