Aledade is the largest network of independent primary care in the United States, helping medical practices transition from volume-based to value-based care models. As an Account Manager, you will serve as the strategic partner and business coach for a portfolio of medical practices in Northern California and the Seattle, Washington area. You will own the day-to-day relationship with physicians, healthcare executives, and practice administrators, diving deep into performance data to identify gaps and opportunities. Your core responsibilities include: **Relationship Management:** Develop and nurture lasting relationships with physicians, providers, and healthcare system executives to support financial and quality performance across all metrics. **Performance Management:** Own comprehensive account relationships across financial performance, quality management, clinical management, population health, care management training, HEDIS and STARs performance, and operational enhancements. Use data analytics tools to monitor practice performance against key objectives and develop project plans to ensure organizational goals are achieved. **Workflow and Staff Coaching:** Train office staff and physicians on processes and technologies (in coordination with Aledade's Value Based Care Curriculum). Provide ongoing assessment of training needs and communicate contract goals and performance outcomes at organizational, site, and provider levels. **ACO Operations:** Coordinate with internal and external stakeholders to develop executive-facing presentations and memos covering practice performance, financial outcomes/projections, and contract terms. **Strategic Advisory:** Proactively surface root causes of performance issues to central leadership and adapt to evolving organizational needs. This is a remote-based role with a West Coast location requirement. You will travel approximately 1 day per month per account (20–25% annual travel) for 2–3 day on-site visits to partner practices in Northern California and the Seattle area. Exact schedules flex based on client needs. **Requirements:** - 5+ years of executive-level communication experience (written and verbal) - Demonstrated knowledge of value-based care contracts across all patient types - 6–8 years of relevant experience in practice management, project management, account management, analytical problem-solving, and strategic healthcare analytics - Ability to travel regionally for multi-day client visits **Preferred Qualifications:** - Bachelor's degree from an accredited college/university in nursing, social work, health technology, or IT-related programs (or 8+ years of significant medical practice management experience with substantial clinic administrative background) - Excellent organizational, project management, and team-building skills - Familiarity with process mapping and workflow analysis tools - Advanced proficiency in PowerPoint, Excel, data visualization, and willingness to learn additional software
