Salary: USD 85,000 - 100,000 / annual
Daymark Health is a value-based oncology company redefining cancer care through a comprehensive, personalized platform that combines dedicated care navigation, symptom-focused support, behavioral health, and social resources with evidence-based interventions and a hybrid in-person + virtual care model.
As an Oncology Nurse Navigator, you will be a key member of an integrated Care Team including a Medical Director, Nurse Practitioner, Community Health Worker, Enrollment Specialist, and Social Worker. You will provide triage, case management, and care navigation for oncology patients, many with additional comorbidities, working closely with Clinical Operations to ensure optimal care pathways are identified and followed.
Key responsibilities include:
• Clinical Support: Provide consultative support over phone, email, or video; be available for urgent clinical escalations.
• Symptom Triage: Triage responses to Symptom Survey and inbound reported symptoms, supporting patients directly or escalating to Nurse Practitioners as needed.
• Care Navigation: Proactively identify and address patient needs including care coordination, nutritional support, discharge planning, and provider referrals.
• Patient Education: Help new patients understand their diagnosis, Daymark's care offerings, and next steps.
• Relationship Building: Establish strong, trusting relationships with patients and care networks through empathy and active listening.
• Integrated Care Coordination: Collaborate with clinical team (Nurse Practitioners, Medical Directors, Social Workers) and non-clinical Health Partners to address social determinants of health such as food security and transportation.
• Operational Excellence: Adhere to Daymark policies, procedures, and documentation standards; collaborate with Clinical Operations on effective use of systems and technology.
This is a full-time, remote role, Monday–Friday, 8:30 AM–5 PM local time. Note: Preference may be given to candidates able to commute to the local office once weekly for team meetings.
REQUIREMENTS:
• Bachelor of Science Degree in Nursing (required)
• Unrestricted Registered Nurse (RN) license (or ability to obtain); licensure in Rhode Island required (or willingness to be licensed in RI)
• Minimum 3 years of nursing experience, including 2 years in oncology nursing
• Passion for helping patients understand palliative care benefits
• Oncology-related certifications strongly preferred or to be obtained within 2 years: Oncology Certified Nurse (OCN), Advanced Oncology Certified Nurse (AOCN), Advanced Oncology Certified Nurse Specialist (AOCNS), or Certified Case Manager (CCM)
• Comfort with technology: Slack, Zoom, Google Suite, and EHR systems
• Strong relational skills and ability to establish rapport with diverse populations (providers, clinical staff, patients, families)
• Bias to action; ability to identify and prioritize needs and act on urgent matters immediately
• Patient-centric approach with deep commitment to Daymark's mission
• Excellent organizational and multitasking skills in fast-paced environments
• Effective communication and listening skills
• Comfort with change and ambiguity; proven success in dynamic environments
• Basic Life Support (BLS) certification required; compliance with Employee Vaccination and Testing policy required by start date
