Local Infusion is seeking a Nurse Practitioner licensed in Massachusetts to join its virtual care team, delivering telehealth-based clinical care to patients with neurological conditions, memory disorders, and dementia. This is a patient-facing clinical role where you will conduct patient assessments, develop and manage care plans, and prescribe therapies in collaboration with supervising physicians. You will partner closely with neurologists and the broader care team to ensure continuity of care, educate patients and families on diagnoses and treatment plans, and maintain accurate clinical documentation in the EMR.

REQUIREMENTS:
- Active, unrestricted Nurse Practitioner license in Massachusetts (required)
- Certification in geriatrics, adult-gerontology, or family practice (preferred)
- 2+ years of clinical experience in neurology, memory care, and/or dementia care settings
- Strong clinical judgment and clinical documentation skills
- Ability to manage a full patient panel independently with physician collaboration
- Excellent communication skills with patients facing cognitive decline and their caregivers
